Parktree Community Health Center provides comprehensive and integrative medical, dental, and behavioral health services to the local community in the Pomona and Ontario areas.
We are looking for an energetic and professional Full-Time Clinical LVN to join our team.
Job Summary: The License Vocational Nurse (LVN) will act as a clinical extension for providers by carrying out clinical duties for patient care. The LVN is a key member of the health center’s care team and works cohesively with other providers, LVNs, MAs, and other staff members. The LVN performs all LVN duties and coordinates and monitors duties. Ensures coordination of patient care services for patients across the continuum of care through collaboration with the patient and family in achieving optimal patient outcomes. This position requires a high level of organization and coordination. Independent judgment is required to plan, prioritize, and organize a diversified workload with professionalism and discretion. The LVN will demonstrate poise and diplomacy to a diverse group of internal and external customers. Participates and promotes patient-centered medical home (PCMH) principles, aiding in the development of office workflows for the medical home, and collaborating with physicians and care teams in developing a standard of care with preventative services and chronic disease management. The LVN will be responsible for vaccine clinics, nurse visit schedule, and the patient portal, in conjunction with the Clinic Supervisor and reports to Director of Infectious Disease & HIV Prevention Services.
Essential Duties:
1. Maintains strictest confidentiality of patient information. Deals discreetly with confidential information concerning ParkTree affairs and personnel.
2. Addresses patient complaints. Reports patient complaints to the Clinic Manager(s). Reports complaints, as necessary, to Risk Management and Compliance staff.
3. Participates in trainings. Leads trainings, when assigned. Mentors new and current Medical Assistant (MA) personnel.
4. Participates in process implementations and improvements, in conjunction with Clinic Supervisors and Clinic Managers.
5. Explains treatment procedures, medications, diets and other provider instructions to patients.
6. Provides education on HIV and STD screenings and safer sex practices. Provides PrEP and PEP education. Assists patient with resources to access PrEP.
7. Provides translation services when needed.
8. Responsible for keeping rooms stocked, clean, and organized.
9. Ensures contaminated supplies are disposed of appropriately. Disinfects and sterilizes equipment.
10. Consistently adheres to infection control policies and procedures.
11. Maintains vaccine logs. Maintains compliance with vaccine maintenance and reporting.
12. Participates in various vaccine clinics, including COVID 19 vaccine clinics.
13. Responsible for ordering, receiving, storage and handling of vaccines. Responsible for maintaining adequate inventory of vaccines.
14. Monitors for and reports vaccine excursions. Follows procedures for vaccine excursions. Participates in vaccine audits. Maintains compliance with various vaccine programs, such as VFC and VFA.
15. Monitors and ensures response to questions in patient portal, within licensure. Forwards patient portal questions to appropriate party when outside of scope of practice. Fields medication refill requests and calls from pharmacy, and routes to appropriate provider.
16. Monitors the nurse visit schedule. Performs nurse visit duties, including but not limited to, TB readings and placements, vaccinations, PT/INR, BP checks, blood draws, rapid HIV testing, and education on medication administration for insulin and asthma medications.
17. Responsible for monthly inventory of medications and supplies, including dispensary medications. Ensures proper disposal of expired medications.
18. Assists care teams, when needed, to ensure adequate back office flow.
19. Operates electrocardiogram (EKG) and other medical equipment, within scope of practice. Administers routine screening tests, within scope of practice.
20. Gives intra-dermal and intramuscular injections and inhalation treatments under the direct supervision of a licensed practitioner.
21. Provides wound care, when needed.
22. Develops and provides educational programs for patients and families in the areas of, including but not limited to, diabetes, hypertension, asthma, HIV prevention, safer sex practices, family planning, vaccine education, and medication use/compliance.
23. Assists with health education outreach to patients as well as the community.
24. Reviews medical plan(s) with patient/family. Provides test results, within scope of practice, to patient/family.
25. Promotes a strong productive team environment. Acts as a role model. Creates a team climate characterized by honesty, trust and open communication.
26. Ensures that staff and self are following regulatory requirements. Serves as resource to staff regarding regulatory requirements.
27. Monitors Clinical Quality Measures. Facilitates and monitors PDSA’s (Plan, Do, Study, and Act) and other quality improvement efforts, as assigned.
28. Assists and participates in audit preparations. Upholds standards for various programs in preparation for audits.
29. Serves as a back-up to the lead MA during lead MA absence.
30. Responsible for LVN phone and providing advice to patients regarding questions during business hours. Notifies and tasks provider for critical lab results. Assists with phone coverage for other sites as needed.
31. Works closely with Director of Infectious Disease and HIV Prevention Services to maintain excellent clinical care workflows, troubleshoot issues and provide solutions to problems on the floor.
32. Completes special projects and other duties as assigned.
33. Completes all mandatory trainings and drills.
Job Qualifications and Skills:
1. Unrestricted Licensed Vocational Nurse license, required.
2. Knowledge of medical terminology.
3. High School Diploma and/or equivalent.
4. Minimum of 1-2 years of experience in a medical office, preferred.
5. Current BLS or CPR certificate.
6. Excellent customer service, leadership and motivational skills.
7. Excellent ability to motivate, supervise staff in a diverse working environment.
8. Ability to establish and maintain effective working relationships.
9. Must demonstrate strict attention to detail.
