Experienced Intake Care Coordinator needed. Candidate must know HCPC, ICD10 & CPT codes. Will be responsible for taking Infusion Therapy and Home Health referrals from referral sources. Must be capable of verifying benefits, entering patient demographics into the computer system, communicating with other departments regarding the status of referrals, and notifying patients/families regarding coverage and payment responsibilities. Ensures that insurance verification is completed, and authorizations are obtained before giving the referral to the Pharmacist.
Work Schedule: Monday to Friday 8 am - 5 pm, or 10 am - 7 pm, with rotating weekends (comp time given). Position can be remote, in-office, or hybrid.
