Job Description:
Outpatient Clinical Documentation Specialist, is responsible for concurrent and retrospective review and ongoing audits of outpatient records for accuracy of diagnosis and procedure codes. Proficient knowledge in: Observation, Hospital Ambulatory Surgery, Complex Hospital Outpatient Visit {Cardiac Catheterization (Percutaneous Coronary Intervention) Lab, Interventional Radiology}, Emergency & Trauma Departments, and other select OP records is a must!
Position requires clinical knowledge of acute and chronic conditions (HCCs, CCs, MCCS), and procedure coding knowledge in documentation opportunities that can directly influence quality of care and severity of illness of our patients.
Interacts with physicians through established query process in order to clarify documentation supporting accurate patient diagnostic and procedure coding.
Provides outpatient education and training to providers, hospital staff and clinical managers on documentation and billing related issues.
May participate in committees and work groups.
May be responsible in creating visual aids, tips, brochures, and formal presentations to providers and assist in training efforts regarding outpatient initiatives.
