What You Will Do to Change Lives

Serves as the primary support and case manager for individuals with mental illness by assisting them to live as independently as possible in their chosen community, focusing on substance abuse and addictions

Shifts Available:
Full-Time; PM 3:00 pm - 11:30 pm; Wednesday - Sunday

Wage range $23.47 - $34.53 – Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have):
High School Diploma or G.E.D
Certified Advanced Alcohol and Drug Counselor (CADC), or Certified Addiction Treatment Counselor (CATC), Substance Use Disorder Certified Counselor (SUDCC, SUDCC II, III, IV)
Six (6) months’ experience in addictions setting
All certifications must be maintained for duration of employment
Within two (2) months of employment, ability to apply financial requirements for billing and learn billing codes
Willingness to transport clients in company van

Essential Functions

Collaborates with members served to create Recovery Plan goals and ensure all services described in the Recovery Plan are provided
Consistent with expertise, provides specialized services and interventions related to substance use/addictive disorders, including thematic groups, evidence-based practices, and collaboration with external agencies and resources
Facilitate groups per the needs and interests of members, especially in the area of education and skill building related to Substance Use Disorders
Understands assessment tools and Harm Reduction Model
Provide and document, quality and quantity of direct services as required by program standards
Coordinates transitions to lower levels of care
Maintains awareness of community resources and assists members served in appropriate use of community resources
Assists members served with all needs, including Activities of Daily Living
Assists members served in making and getting to essential appointments
Assists and advocates for members served in obtaining appropriate benefits

Wellspring Center aims to stabilize individuals in acute crises in order to eliminate or shorten the need for inpatient hospitalization. Wellspring Center will provide intensive and structured psychosocial recovery services 24/7 in a non-institutional, welcoming, safe, and respectful environment.
