The Alcohol & Drug (AOD) Case Manager provides trauma-informed, culturally competent support to formerly incarcerated and/or unhoused women in recovery. This hybrid position integrates substance use counseling with intensive case management to help clients achieve long-term sobriety, housing stability, and self-sufficiency. The AOD Case Manager is responsible for assessing client needs, developing individualized treatment and housing plans, providing clinical counseling, and connecting clients to community-based supports, in alignment with DHCS, CARF, and Time for Change Foundation’s programmatic goals.

Key Responsibilities

Clinical & Case Management Services

  • Conduct biopsychosocial assessments and develop individualized treatment and transitional action plans using ASAM criteria and housing needs assessments.
  • Provide individual and group counseling using evidence-based practices such as CBT, MI, and trauma-informed care.
  • Monitor client progress through weekly check-ins, service coordination, and documentation.
  • Assist clients in navigating and accessing social services, including mental health care, public benefits, education, employment training, and permanent housing.
  • Maintain active engagement in recovery services including 12-step groups and sponsor accountability. • Provide crisis intervention, relapse prevention, and de-escalation support as needed.
  • Advocate for clients with external agencies and accompany them to appointments when appropriate.

Administrative Duties

  • Maintain timely, complete, and confidential documentation in alignment with HIPAA and DHCS standards (e.g., clinical notes within 24 hours).
  • Track client milestones and update treatment/housing plans monthly. • Participate in quality improvement activities and required staff trainings (HIPAA, CPR/First Aid, cultural competence).
  • Maintain accurate electronic and physical case files and contribute to outcome data reporting. Program Collaboration & Development
  • Attend multidisciplinary team meetings and case conferences to review client progress.
  • Coordinate care with medical providers, behavioral health professionals, and housing specialists.
  • Contribute to development of new treatment and recovery support protocols.
  • Participate in outreach, community education, and prevention activities. Minimum

Qualifications

  • Certified AOD Counselor registered with CCAPP, CADTP, or CAADE (active and in good standing).
  • Bachelor’s degree in Social Work, Counseling, Psychology, or a related field.
  • At least 3 years of direct experience working with substance use disorders, incarceration-impacted populations, and/or co-occurring disorders.
  • CPR/First Aid certification and current TB clearance. • Proficient in Microsoft Office Suite and electronic health record systems.
  • Strong knowledge of DSM-5, ASAM, trauma-informed care, and harm reduction strategies. Time for Change Foundation is an at-will Equal Opportunity Employer.

Job Type: Full-time

Pay: $26.44 - $33.00 per hour
