Essential Duties and Responsibilities include the following:


  • Assists ACP/Case Mgr. with advance care planning activities.
  • Interfaces with the inpatient, SNF, Outpatient, prior auth team, & hospice teams to coordinate advance care planning activities.
  • Receives and logs new hospice referrals into hospice log in real time.
  • Attends inpatient rounds/ICT Meetings, as requested/needed by ACP CM to receive potential hospice care referrals
  • Supports ACP CM with referral tracking and updating hospice log on a daily basis.
  • Conducts follow-up calls for members who discharged/revoked from hospice when warranted.
  • Follows up with Hospice Providers for members who discharged/revoked to re-convert with provider as appropriate.
  • Conduct follow-up calls for members referred and assist with scheduling home visit appointments for ACP/Case Mgr.
  • Communicate with PCP members referred for Hospice if received from other sources.
  • Communicate with PCP office to obtain order for hospice and faxing to hospice provider.
  • Requests medical records from medical office or hospital as needed and upload into member's chart to assist in ACP member review.
  • Follows up with Hospice Providers to ensure service issues are addressed.
  • Verifies eligibility and benefits as required
  • Processes any identified referrals when member is discharged from Hospice for members as needed.
  • Documents all care coordination on member's case/ medical record.
  • Keeps patient and/or family members or other customers informed of any changes.
  • Assists other staff and supports the team approach.
  • Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner.
  • Demonstrates honesty and integrity in everyday activities.
  • Protects privacy for both patients and employees; ensuring all personal health information is kept confidential i.e., (sensitive papers, charts, and reports are not in view of the public).
  • Recognizes when an error has been made and immediately reports to the ACP CM or Director, Social Services as needed.
  • Participates in “service recovery” through follow-up with an upset customer, gathering information, and demonstrating empathy.
  • Treats all customers (Members, Peers, Vendors, and Hospitals) with respect and dignity.
  • Interacts professionally with other healthcare service providers.
  • Communicates appropriately and clearly to management, co-workers, and physicians.
  • Consults other departments as appropriate to provide for an interdisciplinary approach to the patient's needs.
  • Performs other duties as assigned.
  • Participates in the efficient, effective, and responsible use of resources such as supplies and equipment.
  • Maintains all reference material that is provided by the ACP CM or director.
  • Assist with any special projects
  • All other duties as directed by management.

The pay range for this position at commencement of employment is expected to be between $21 - $23 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

 

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements:

Education and/or Experience:


  • High school graduate required; Associate in Arts degree preferred.
  • Hospice experience strongly preferred.
  • Knowledge of Healthcare and Managed Care preferred.
  • Typing 60 words per minutes with accuracy.
  • Knowledge of computers, faxes, printers and all other equipment.
  • Proficient in MS Office programs (i.e., Word, Excel, Outlook, Access and Power Point).
  • Ability to deal with responsibility with confidential matters.
  • Ability to work in a multi-task, high stress environment.
  • Medical terminology, ICD10, and HCPC coding.
  • Good phone etiquette required.
  • Knows and follows the Employee Handbook policies and procedures.

Compensation: $21.00 - $23.00
