Prototypes, located in Pomona, offers residential and outpatient substance use disorder (SUD) treatment and mental health services Prototypes is a leader in the field of SUD services, community mental health and criminal justice treatment. Prototypes installs hope that recovery is possible. This position is for our DMC funded, outpatient substance abuse treatment program.
The Care Coordinator II is a registered substance use disorder professional who maintains registration from an approved/accredited California agency. The Care Coordinator II is responsible for delivering services to a caseload of clients and providing individualized substance use disorder services. The Care Coordinator II works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360’s philosophy, goals, policies, mission and vision. The Care Coordinator II may provide treatment utilizing different modalities including individual and/or group. Services may be provided in: outpatient office, residential treatment, or the field, based on client and program need.
Key Responsibilities
Assist clients with intake, assessment, and orientation to program upon admission or transition in care level.
Help lead the client through the treatment plan and delivers individual and group substance use disorder services.
Monitor client's progress toward achieving treatment plan goals and provides treatment plan input.
Utilize trauma informed care while applying motivational interviewing, cognitive behavioral therapy, and evidence-based practices.
Deliver residential and intensive outpatient service clients with a minimum of 1 individual session per week. Deliver outpatient clients a minimum of 1 session every two weeks.
Seek clinical consultation throughout the treatment episode and when encountering new and/or high risk clinical circumstance.
Work collaboratively with other members of the treatment team including the Case Manager, Clinical Therapist, Medical, and Psychiatrist.
Maintain frequent communication with the treatment team and engages in regular consults, clinical team, case conferences, and plan development meetings.
Deliver services in outpatient, residential, or field depending on assignments and client need.
Responsible for treatment oversight of the clients on their caseload.
Education and Knowledge, Skills and Abilities
Required:
Registration as a Substance Abuse Counselor from an approved/accredited CA agency AND 3 years of SUD experience.
High school diploma or equivalent.
Experience working with clients experiencing acute withdrawal from substances.
Desired:
Preferably minimum of two years’ experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery.
Experience delivering evidence-based practices.
