Title: Case Manager/Counselor
Department: Clinical
Supervisor: Program Manager
Position Status: Full Time/ Part Time

Job Summary: Works with and manages client cases from intake to discharge, follow client progress, keep
Program Manager informed of any issues/problems with any client.

Duties and Responsibilities:
1. Maintains caseloads and client treatment processes: a. Responsible for facilitating group and charting. b. Manage and maintain treatment plans, problems list, progress notes and all other required client documents pertaining to treatment. c. Maintains contact with client’s family on a weekly basis, discussing progress/problems depending on client’s release of information. As appropriate and document integration d. Assures proper client consents and that other treatment documents are completed e. Maintains contact with clients’ on a regular basis, provides one-on-one sessions with clients’ one-two times a week. f. Responsible for charting one-on-one sessions, groups, and any and all incidents, accidents, and incidental pertinent information relating to client care and treatment. g. Meets with clients on an individual basis when necessary. h. Corresponds with client’s probation officer and the courts if applicable i. Ensures clients outside resource appointments are scheduled, arrange transportation, communicate and coordinate with all required staff j. Maintains contact with Clinical Director/Program Manager regarding caseloads. k. Establishes an individual assessment and treatment program. l. Ensures clients attend and participate in all required groups and complete all related assignments in a timely manner. m. Responsible for attending and participating in mandatory weekly staff meetings to discuss client care and all aspects of administration. n. Knowledge of policy and procedure manual relating to program protocol. o. Review resident files on a regular basis to ensure quality assurance.

2. Case Manager performs and maintains professional conduct in the following areas: a. Maintain and adhere to all Federal, State, and County regulations regarding client confidentiality and staff code of ethics. b. Perform tasks the Program Manager assigns including, but not limited to, schedule, case load, and deadlines. c. Continuous training in areas where needed, attendance at seminars / continue earning CEUs. d. Treatment planning is to be kept current and be cohesive with the needs of the client. e. Client education—provide current alcohol/drug information and available resources to clients, groups, and families.

Job Type: Part-time

Pay: $25.00 - $28.00 per hour
