Position Overview:

Social Work Action Group (SWAG) is looking for a Certified Alcohol and Drug Counselor that is motivated, compassionate, and is a committed team player with strong interpersonal and communication skills. The successful candidate will be organized, adaptable, efficient, and able to prioritize multiple tasks in an evolving environment. The successful candidate must be willing to conduct the majority of their work in the field and not in a traditional office setting.

Under the supervision of the Program Coordinator, the Alcohol and Drug Counselor will perform duties and responsibilities outlines below, as well as other duties as assigned. Must be familiar with the organization, stay abreast of changing situations to identify problems, gather data, develop and assess alternative courses of actions.

Typical duties may include, but are not limited to, the following:

  • Complete intake assessments, treatment plans, documentation, progress notes, and case management according to established legal and ethical codes and in compliance with Social Work Action Group.
  • Providing on-site assessment and consultation with primary care practitioner, Licensed Clinical Social Worker, and case manager as required.
  • Provide resource and referral information to clients and link to partnering agencies as necessary for comprehensive treatment and document accordingly.
  • Provide crisis intervention services to clients on the streets, in homeless encampments and other places not meant for human habitation.
  • Participate as a member of the Case Review Team in reviewing client cases.
  • Provide counseling to address presenting needs including crisis intervention services.
  • Complete documentation timely and accurately.
  • Participate in a multidisciplinary team to develop, execute, monitor, and adjust comprehensive and coordinated care plans that include shared goals with measurable outcomes.
  • Assess client needs, abilities, and behavior using a variety of methods including interviews and direct observation of behavior in individual and group settings.
  • Encourage clients to talk about and explore the relationship between thoughts, feelings, and behavior.
  • Teach self-management skills needed for participation in treatment and recovery.
  • Promote compliance with treatment initiated by the care team by offering strategies to cope with side effects of medication changes.
  • Employ evidence-based approaches to shared decision-making that respect the client’s preferences and social norms while targeting optimal treatment outcomes.
  • Encourage clients to take control and responsibility of their health through self-management and use of community resources.
  • Will be responsible for conducting Educational groups, Clinical Intervention, and discharge planning.
  • Must have knowledge of the ASAM criteria.
  • Will be responsible for development of initial treatment plans as well as updates of the treatment plan during recovery process.
  • Will be responsible for documenting progress, individual, and weekly notes in client files
  • Will be responsible for community resource development and referral services.
  • Will have case management knowledge.
  • Will be responsible for keeping up to date UA documentation.
  • Must have knowledge and have the ability to utilize the 12 core functions to AOD counseling.
  • Must maintain updated and current certifications and/or registrations.
  • Will assist the Program Coordinator in the day to day activities as needed and as required.
  • Any other tasks assigned by any supervisor.

Requirements:

  • Have a current valid Drug and Alcohol Certificate issued by the California Consortium of Addictions Programs and Professionals and or the California Association for Alcohol/Drug Educators.
  • Must maintain a working knowledge of all state and county regulations.
  • Must have the ability to multi-task, often changing assignments on short notice. Must accomplish assigned tasks thoroughly with concern for all aspects no matter how minor the detail.
  • Deliver treatment services to clients that are currently homeless and recently housed;
  • Lead and participate in face-to-face group and individual meetings, motivational interviewing, cognitive behavioral therapy and other therapeutic processes;
  • Work directly with participants to develop and implement treatment service plans and connect participants to supportive networks as they transition out of homelessness;
  • Review treatment plans, evaluate and record treatment progress, records visible changes in appearance, behavior and demeanor of clients. Maintain accurate, timely, and confidential participant records; Summarize participants background, treatment plan, and recovery process.
  • Must be sensitive to the feelings and needs of others.
  • Must possess ability to communicate clearly and effectively both orally and in writing; including professional Telephone Etiquette and when talking with individuals and group, as well as clear, concise and professional written communication.
  • Must possess Interpersonal Skills; working well as a team with colleagues and clients to achieve goals and assignments with tact and professionalism.
  • Must have an understanding of the nature of substance abuse and its respective treatment and recovery.
  • Must be free of alcohol and/or other drugs on a daily basis. If a recovering alcoholic or drug abuser, a reasonable amount of current sobriety will be required. Determination to be made by the Executive Director.
  • General computer skills including typing, Microsoft Word, Microsoft Excel, Microsoft Outlook, and medical record data review and entry.
  • Minimum three years of providing Addiction Treatment Services (i.e., assessment, diagnosis, individual and group therapies) to homeless, mentally ill, substance addicted adult populations.
  • Able to work in a high tolerance harm reduction model with homeless clients who have multiple barriers.
  • Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision.

Required Qualifications

  • Employment Eligibility Verification.

  • Updated tuberculosis test.
  • Successful completion of background screening.
  • CPR/First Aid training.
  • Driving is an essential function of this position
  • Must have Valid CA Driver's License
  • Must provide proof of insurance coverage
  • Must be able to qualify for SWAG insurance coverage

The employee will be part of a multi-disciplinary team that pro-actively engages and encourages homeless individuals to accept and receive services to end their cycle of homelessness, in areas not meant for human habitation. These areas may include homeless encampments in lake and river bottoms, trash enclosures, abandoned buildings, etc. The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, lack of personal hygiene and environmental conditions associated with field work. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
