Share Our Selves (SOS) is proud to be the largest community health center in Orange County to combine wrap-around social services with health care. Our values of dignity, justice, service and excellence are translated through every service offered at SOS. We currently operate six health sites located in Costa Mesa, Santa Ana, Mission Viejo, Anaheim and Newport Beach; dedicated to improving health outcomes of low-income and at-risk populations in Orange County.

We have an immediate opening for a Community Health Worker/Pharmacy Technician. This position will have a delivery component within Orange County and the best fit candidate must be able to work in pharmacy AND facilitate in home visit to deliver medication/medical aids and educate patients.

The pay rate offered will be based on license, experience and skills within the range posted.

JOB SUMMARY: The Community Pharmacy Health Worker (CPHW)is a licensed pharmacy technician that assists the SOS pharmacists with assigned duties under the direct supervision of the Clinical Pharmacy Manager; performs “nondiscretionary tasks” including: removing drug from stock, count or mix, place in container, affix the label and package and repackage medication; completes documentation and data entry as required; maintains cleanliness and order in the facility throughout the work day; assists patients as authorized. Manages medication access programs and assists pharmacist with clinical pharmacy services. The CPHW provides high quality patient care that extends beyond the doors of the SOS Pharmacy and Medical Clinic. This position assists in community health activities, including but not limited to: discharge medication reconciliation, comprehensive medication therapy management review, and disease self- management education, and small group patient education workshops to assist participants in safe medication use. The CPHW assists with scheduling of post-discharge follow-up visits in patient’s home. Under the SOS pharmacist’s guidance, performs a variety of technical duties related to preparing and dispensing drugs in accordance with standard procedures and techniques of a licensed pharmacy technician. The CPHW has sufficient transportation, vehicle insurance and DMV record to drive personal vehicle for delivery of medications or medical supplies to patient home. The Community Pharmacy Health Worker works in the team-based model of care.

JOB RESPONSIBILITIES:

Community Pharmacy Health Work

  • · Assists in scheduling home visits with patient and/or patient’s caregiver.
  • · Develops rapport with patients and/or patient’s caregiver.
  • · Assists in calling patients before visit as a reminder
  • · Delivers via home visit; medication, medical supplies or medical aids.
  • · Educates patient according to program protocol and procedures during the home visit to ensure the participant can recognize and report signs and symptoms of adverse drug events through recognition of worsening symptoms and calls the appropriate agency documented in the program procedures
  • · Educates patient on disease self-management, adherence aids, and adherence counseling
  • · Assists in scheduling visits with the pharmacist and primary care provider
  • · Documents the response of the patient and family to protocol activities delivered.
  • · Communicates observations to the SOS Pharmacist for revision of the individual plan of care, with urgent needs.
  • · Interacts with patient/family and Pharmacist to contribute to the revision of the medication reconciliation process, disease self-management, and care navigation.
  • · Educates and reinforces any new plan of care based on the medication plan of the Pharmacist and healthcare team.
  • · Provides professional, efficient, and courteous service to patients.

Pharmacy Technician in Pharmacy

  • · Generates prescription labels during sessions
  • · Receives, processes, and prepares prescriptions as indicated by SOS policies and procedures and California State Board of Pharmacy.
  • · Ensures all prescriptions are processed and prepared correctly and efficiently.
  • · Assist in ordering, stocking, and maintaining appropriate medication inventories
  • · Maintain compliance with 340B program
  • · Maintain pharmacy in a clean, orderly, and safe fashion
  • · Receives and triages information requests; researches and provides information within scope of responsibility, locates items, or refers inquiries to pharmacist.
  • · Performs technical duties related to documenting patient medication history and any patient reported problems with medication therapy in accordance with standard procedures and techniques.
  • · Schedules patients for Comprehensive Medication Management visits.
  • · Provides professional, efficient, and courteous service to patients.
  • · Record refrigerator and freezer temperature twice daily
  • · Answer phone calls and retrieve messages from pharmacy and refill lines
  • · Assist in completion and follow up of Prior Auth process
  • · Computer data entry for grant and collaborative requirements
  • · Undertake administrative tasks as assigned.
  • · Organizes medication transport to various clinic sites
  • · Maintains and promotes compliance with all regulatory requirements, reimbursement standards, organizational standards, and policies and procedures related to area of responsibility.
  • · Practices principles of patient safety by following policies and procedures developed to promote medication safety and practice standardization.
  • · Consistently achieves high standards of accuracy in work product and documentation.
  • · Practices universal precaution procedures and proper safety techniques in accordance with hospital and departmental policies and procedures. Immediately reports any safety issues to appropriate person

EDUCATION AND/OR EXPERIENCE:

  • · High School Diploma or GED equivalent.
  • · CA Pharmacy Technician license required
  • · Minimum One (1) year experience as a Pharmacy Technician
  • · Bilingual – English and Spanish strongly preferred
  • · Basic understanding of pharmacy management software;
  • · Demonstrated ability to develop and maintain positive relationships with department staff
  • · Demonstrated excellent customer relations and communication skills.
  • · Ability to communicate effectively both orally and in writing with proper spelling/grammar
  • · Ability to establish work priorities to meet established deadlines
  • · Must be detail oriented and work in a fast paced environment
  • · Ability to document proficiently as per pharmacy technician guidelines
  • · Knowledgeable of medical terminology, drug nomenclature, symbols and abbreviations associated with pharmacy practice
  • · Must be familiar with HIPAA laws with regards to pharmacy
  • · Strong computer skills using Microsoft Word, Excel and Outlook
  • · Must be able to drive, have reliable transportation, maintain DMV record with no more than 2 points (non major infractions), maintain vehicle insurance and be insurable under SOS company driving insurance.
  • · Must be comfortable entering SOS patient home for visit, delivery and education of patient and/or caregiver.
  • · Must be able to follow SOS infection control and bloodborne pathogen policy including in home visits
