JOB SUMMARY

The Wraparound Facilitator conducts clinical assessments, provides diagnosis, develops client plans, facilitates child-family team meetings, collaborates with treatment providers, family members, and other supports to client health improvement in the home and community.


RESPONSIBILITIES

  • Provide individual, family, and group assessment/therapeutic interventions utilizing strength-based approach.
  • Facilitate Wraparound meetings and respond by phone to clients and families as the primary contact and case manager.
  • Collaborate and coordinate with other service providers to provide care and meet client plan goals.
  • Complete Cerner documentation, progress notes and client plans. Write safety plans, discharge summaries and high-risk assessments, and complete other program/County required documentation
  • Ensure client medical records meet program/county quality assurance expectations.
  • Attend supervision, case consult, team meetings.
  • Creates plan for 24-hour emergency crisis intervention response and support and provides crisis management services as needed.
  • Regularly drive to off-site meetings and assessments in family homes, schools and other agency locations.
  • Coach team members.
  • Comply with standards of care established for the program. Continually demonstrate the ability to provide quality care to program clients.
  • Comply with all MHS and program rules, policies and procedures including HIPAA standards.


QUALIFICATIONS

  • Active Board of Behavioral Sciences registration as Associate or Licensed MFT, PCC, or SW.
  • Master’s Degree in Social Work, Counseling, Psychology or other field related to BBS licensure.
  • Minimum of one year of experience working with children, youth, and their families in a mental health setting.
  • Ability to create and implement individual case plans.
  • Experienced, effective counselor providing counseling, therapy and clinical services to individuals, groups, and families.
  • Excellent oral and written communication skills. Ability to write clear, concise documentation and input into electronic health records system.
  • Excellent organizational and time management skills.
  • Excellent writing skills.
  • Proficient user of Microsoft Office Suite and EHR software.
  • Possess strengths in the areas of behavioral theory, clinical theory/intervention, child/family systems.
  • Must be able to engage culturally diverse populations.
  • Ability to provide effective counseling, therapy and clinical services, and the ability to successfully work as a member of a treatment team.
  • Ability to collaborate with team members to support client needs.
  • Understanding of the distinction between therapeutic and social relationships.
  • Ability and desire to maintain positive morale and unit cohesion as evidenced by maintaining a cooperative and flexible attitude toward coworkers, showing adaptability to change, exhibiting effective communication and interpersonal skills, and taking initiative to solve problems.
  • Ability to problem solve and react appropriately to emergency situations
