Full Job Description

Introduction: The Housing Solutions Case Manager under the direction of the Housing Solutions Manager is responsible for ongoing needs assessment, personal treatment plans, implementation, support, information, referral, and advocacy for individuals/families that are part of the Mercy House Permanent Housing Programs. This position requires successful working relationships with residents that support their ability to increase their self-reliance. This position will influence and recognize positive behavior changes and will identify problems and obstacles that threaten to undermine residents’ self-reliance and housing stability. Maintaining accurate files and records is essential to this position. This position will ensure that the team is practicing trauma informed and client focused strategies.

Qualifications: Bachelor’s Degree in Social Work or related field, with experience working in chemical health, mental health, and co-occurring disorders is highly preferred. Candidates with less than a BA/BS degree must have at least 2 years of direct life experience working with long-term homeless, low income, and diverse populations and have a working knowledge of mental health and crisis interventions are welcome to apply. Excellent verbal and written communication skills and the ability to work independently are required. Valid CA Driver License, proof of vehicle insurance and reliable transportation is required.

Ability to: Work effectively with a diverse homeless population; plan, organize and prioritize duties; speak effectively before small groups; perform crisis interventions as needed; clearly communicate information and instructions verbally and in written form; maintain a positive, professional and safe environment while on duty; and establish and maintain effective working relationships with others.

Dependability, responsibility and the ability to communicate effectively and respectfully are required skills. Desired skills include strong written and verbal communication skills, creative thinking skills, attentiveness to details, and organizational skills. Employee should be proficient in all Microsoft Office programs. Fluency in Spanish is a significant value.

I. Client/Service Delivery:

  • Maintain a minimum caseload of 22-25 individuals. Meet with Permanent Housing Residents as needed to develop and monitor each individual’s case plan.
  • Utilize harm reduction interventions and methodologies to provide client centered services to keep them in housing.
  • Utilize skills regarding crisis prevention and intervention.
  • Practice Trauma Informed Care when interacting with clients
  • Assist residents to build upon their strengths and recognize their needs in a respectful manner
  • Visit and inspect resident apartments to ensure that they are properly maintained.
  • Be knowledgeable of community resources and make referrals as needed.
  • Organize and facilitate social and recreational activities designed to promote community involvement and peer support
  • Work with volunteers and community resources to assure that residents are linked with services such as employment, education, tutoring, and/or mentoring as appropriate.
  • Attend trainings that increase knowledge of effectively meeting clients’ needs (including co-occurring disorders, harm reduction, and cultural competence).
  • Facilitate relationship building between other service providers and clients.
  • Provide comprehensive Case Management services including initial needs assessment, benefit assessment, housing objectives, tenant education, advocacy, and referrals.
  • Develop an individual case management plan, including financial planning, and housing goals with recipient individuals and families
  • Create schedule and meet regularly with recipient individual and/or family unit, tracking progress towards goals in detailed case files with charts, case notes, and outcome evaluation
  • Assist clients with employment searching and housing related issues.
  • Follow all Case Management Policies and Procedures

II. Team/Staff Relations

  • Participate in the emergency on call schedule to ensure clients’ needs are met.
  • Attend regular internal and external agency meetings
  • Work as a Mercy House team member with all staff and volunteers in order to implement the mission and core values of the agency
  • Attend staff meetings and training workshops as assigned
  • Attend monthly meetings with the Housing Solutions team to discuss client progress, needs and share resources
  • Work as a team to plan and develop resident programs.
  • Accept the responsibility for aiding the development of a positive team relationship with other Housing Stability Specialists and Supervisor

III. Administrative

  • Keep accurate, up to date documentation on all clients, including but not limited to stabilization plans, incident reports, case notes, etc.
  • Assist with any and all data collection systems
  • Ensure the confidentiality and safety of case files and resident information
  • Ensure documents submitted on behalf of the client are valid and submitted on a timely manner
  • Willingness to gain knowledge and understanding of Adult Rehabilitative Mental Health Services and Substance Abuse Treatments
  • Other duties as assigned by Supervisor

Hours: Monday-Friday 9am-5pm, will include some weekends and evenings, must be able to respond to crisis situations and client emergencies as necessary.
