Job Title: Intake Coordinator
Reports to: Clinical Supervisor
Job purpose: The intake coordinator will conduct initial assessments for all newly admitted patients. Assessments include conducting one-on-one interviews with patients collecting data for the Addiction Severity Index (ASI) and Interpretive Summary, gathering information and entering it into The California Outcomes Measurements System (CalOMS) data collection system. The intake coordinator will also maintain current patient files by updating all urine drug screens, updating all patient dosing records, completing all patient discharges, and assisting front office manager with checking in patients for dosing.
Responsibilities:
a. Conduct interviews and case management services with new admits within seven (7) days of initial intake to collect information regarding history of substance use; biopsychosocial history and current stressors; patient strengths, needs, abilities, and preferences; legal stressors; family history of substance abuse and mental health issues; and determination of level of care.
b. Enter all data collected at time of interview into the California Outcomes Measurements System (CalOMS) data collection system.
c. Enter all data collected at time of interview into the Addiction Severity Index (ASI).
d. Enter all data collected at time of interview into the Interpretive Summary.
e. Place CalOMS, ASI, and Interpretive Summary into patient file and submit to Clinical Supervisor for review and signatures.
f. Update all patient Master Urinalysis Report forms located in patient file monthly.
g. Update all patient dosing records monthly.
h. Assisting Front Office Manager with:
1. Checking patients in for dosing
2. Answering telephone calls
3. Auditing and collating charts for quality assurance
Qualifications:
Candidate must be Certified with CCAPP, CAADE, or CADTP, or registered with BBS.
a. Minimum of intern status as a substance abuse counselor with any California approved substance abuse certification body. Certified with CCAPP, CAADE, or CADTP. Applicants registered with BBS also considered.
b. Minimum six (6) months experience working in a substance abuse treatment program.
c. Experience with conducting patient intakes.
d. Basic understanding of computer word processing.
e. Minimum 30wpm typing skills.
f. Possess good time management skills.
g. Conduct self ethically and professionally at all times.
h. Has knowledge of and is culturally sensitive to the problems of the patients of this clinic.
i. Perspective individual will not display any evidence of alcohol or drug/medication misuse and/or unlawful behavior as an employee of SOAP MAT, LLC.

Pay: $25.00 - $27.00 per hour
