Must Be Licensed or Certified Counselor
MINIMUM 2 YEARS EXPERIENCE AS SUD COUNSELOR
Basic Duties and Responsibilities:
Schedule :
Monday 1pm - 6 pm
Wednesday 1pm - 6 pm
Friday 1pm - 6 pm
Saturday 1 pm - 6 pm
Provide group therapy to clients presenting with substance use disorders and dual diagnoses.
Document group notes following professional protocol, and complete all notes within 24 hours after a group is held.
Follow through on all directives provided by Clinical Director, Program Director and collaborate with other therapists/staff as needed.
Attend any relevant staff meetings and in-service training.
Maintain professionalism in interactions with staff and clients at all times and exercise maturity of judgment. Intakes, assessments, weekly schedule,
Maintaining patient records related to screening, intake, treatment, continuing care, progress toward goals and objectives, discharge summary, and treatment outcomes that are accurate, concise, timely, and useful for coordinating treatment team planning, monitoring, and review.
Providing homework assessments, collecting surveys, providing individual and group sessions, providing case management to patients on weekly basis Maintaining patient records related to screening, intake, treatment, continuing care, progress toward goals and objectives, discharge summary, and treatment outcomes that are accurate, concise, timely, and useful for coordinating treatment team planning, monitoring, and review.
Conducts individual counseling sessions, as scheduled and documents as required; Conducts group, individual, and family sessions, create a schedule for the week, complete intakes and all necessary documents as required; GIRP/ GIRP notes facilitate assigned individual, group or family sessions; Group Facilitate groups for Anger Management, Domestic Violence, Project Fatherhood, and Health & Wellness, Mindfulness/Meditation, Relapse Prevention, Drug Education, Living in Balance, Relationships in Recovery, Parent Education, INS/Group Counseling, Non-12-Step Program, Trauma, etc
Monitoring patient progress and making changes to the therapeutic approach and best practice, evidenced-based interventions based on patient response to treatment Working collaboratively with the patient and appropriate others to develop a comprehensive treatment plan that addresses priority needs of the patient, desired treatment outcomes, an agreed-upon plan of action, and reassessment;
