POSITION TITLE:
 Licensed Vocational Nurse (LVN)

 Registered Nurse (R.N.)

 


POSITION OVERVIEW:
Under the direct supervision of the R.N., responsible to assist physicians, advanced practitioners, and R.N.’s in the provision of medical care services to patients based on duties within the L.V.N. scope of practice (e.g., typically include, but are not limited to, provision of basic hygienic and nursing care; measurement of vital signs, etc.). Provides direct patient care nursing services throughout the five stage life cycle, OB/GYN, pediatrics, adolescent, adult including women’s health, and geriatric medicine, performs nursing procedures as ordered by FHCCGLA’s providers.
Responsibilities:
DUTIES/RESPONSIBILITIES:
  • Responsible for taking vital signs: blood pressure, pulse, respiration, temperature, weight/height measurements, obtains head circumference for patients two (2) years and under, obtains patient’s clinical complaint(s), conducts medication reconciliation, triages patient using triage protocol in the absence of an R.N. Responsible to document all the above information in the patient’s electronic health record (E.H.R.).
  • Responsible for performing screening tests, including e.g., vision, audiogram, Snellen test, EKG, pulmonary function test and all other screening tests within scope of practice as ordered.
  • Responsible for administering medications, injections and immunizations to patients per providers’ orders, and complies with immunization and medication administration handling, and storage policy and procedures.
  • Responsible for assisting physicians and mid-level providers with pelvic examinations and minor procedures following informed consent, infection control and other related policy and procedure.
  • Responsible for performing basic laboratory procedures to include, obtaining urine/blood specimens (phlebotomy) for testing, Hemoglobin, urine analysis, pregnancy tests, throat cultures, etc.
  • Responsible for providing and documenting health education given to patients under the Patient/Family Education in the E.H.R.
  • Responsible for assisting patients with completing WIC vouchers and other health and human services forms related to health care delivery services.
  • Responsible for receiving and documenting messages pertaining to patients in the absence of FHCCGLA’s providers and R.N., and informs providers, R.N. or immediate supervisor in a timely manner within the work shift to ensure timely call-back or follow-up.
  • Responsible to assist as needed with maintaining daily equipment and supply logs as well as for equipment sterilization and health screening logs.
  • Responsible for equipment sterilization as assigned and follows safety and infection control policy and procedures. Shall also report any issues to immediate supervisor if steam sterilizer is not working appropriately, etc.
  • Undertakes continuous self-improvement, attending applicable training, seminars, in-services and educational classes to maintain nursing skills competency and current knowledge for standard of care and effective practices (Rev. 3/94, 8/07, 4/13, 8/15, 1/16 2 Licensed Vocational Nurse).
  • Responsible for following all agency safety and health standards, regulations, procedures, policies and practices.
  • Assists with data collection for various programs.
  • Ensures program compliance by adhering to the regulations and standards set forth by Vaccines for Children (VFC) Program.
  • Keeps up-to-date on program requirement changes and educates clinical support staff such as Medical Assistants (M.A.’s) of any pertinent changes/updates.
  • Observes and documents competency of clinical support staff (M.A.’s) and provides instruction and feedback on technique.
  • Ensures timely documentation and submission of self-competency skills checklist to R.N.
  • Responsible for the oversight of all vaccines (e.g., VFC, adult, managed care, etc.); and assumes all Immunization Coordinator (I.Z.) duties when necessary (e.g., coverage, audits, etc.) to include:
  • Orders vaccines, enter inventory, maintains logs, and completes required reporting (including reminder/recall).
  • Remains current with all company, pertinent required training; i.e. VFC training; Quarterly Staff; MAC; BLS; etc…
  • Participates in audit readiness activities including entrance/exit interviews and the audit process as necessary including obtaining pertinent training documentation (e.g., sign-in sheet, minutes, notes) for audit Corrective Action Plan (CAP) and/or compliance to ensure audit requirements are met.
    • Participates in FHCCGLA’s ongoing Quality Improvement, Risk Management and Safety activities.
    • Identifies, initiates and implements measures to deliver high quality care to patients and improve services.
    • Responds efficiently and timely to all patient and provider staff needs and inquiries.
    • Ensures excellent customer service to all FHCCGLA patients.
    • Resolves patient issues in accordance with company policies & procedures and healthcare regulations.
    • Works with the operations managers to manage patient scheduling and flow to address bottlenecks, scheduling issues, etc.
    • Handles patient grievances according to FHCCGLA’s Policy & Procedure.
    • Assists with COVID Vaccine administration and all other associated duties.
    • Assists with COVID testing and all other associated duties.
    • Assists in developing, updates & reviews of FHCCGLA Policies & Procedures (P&P’s) as needed (with input from all other key personnel).
    • Ensures HIPAA compliance by maintaining strict confidentiality of all patient data and E.H.R./Practice Management System (PMS) according to regulations and FHCCGLA’s P&P’s.
    • Maintains facilities, equipment and practices in accordance with hygiene, safety and compliance regulations related to OSHA, the California Department of Public Health Radiologic Health Branch (CDPH-RHB), FHCCGLA’s P&P’s, etc.
    • Attends the following meetings/trainings:
    • Mandatory Quarterly Staff Meeting/Trainings- Quarterly (Jan., Apr., Jul. & Oct.)
    • Corporate Risk Management Meeting- Quarterly (Jan., Apr., Jul. & Oct.), as needed (advanced notice will be provided when feasible)
    • Clinical & Operations (C&O) Meeting- As needed (advanced notice will be provided when feasible)
    • Meetings with FHCCGLA’s Executive Leadership, as needed (advanced notice will be provided when feasible)
    • Other pertinent meetings- As scheduled
    • Remains informed of:
      • Current legal and regulatory changes related to scope of practice.
      • Specific programs/payors, insurances accepted, and services being offered at FHCCGLA.
      • All applicable Policies & Procedures
    • All other duties as assigned.
    Qualifications:
    MINIMUM QUALIFICATIONS:
    • Current California Vocational Nurse License from an accredited nursing program.
    • Current CPR Certification- required to be kept up to date.
    • One (1) year nursing experience in an ambulatory outpatient or hospital setting. New graduate LVN’s considered.
    • FQHC experience, highly preferred.
    • Excellent analytical skills.
    • Motivation to take initiative to ensure all tasks performed are completed thoroughly and accurately.
    • Excellent writing and verbal communication skills.
    • Knowledge of administrative principles and procedures.
    • Computer knowledge preferred (e.g., NextGen (E.H.R. & PMS), Microsoft Word and Excel).
    • Bilingual English/Spanish and familiarity with the Hispanic culture.
