About the role

 


Our OP Case Manager supports clients transitioning from in-person to telehealth. You will monitor clients progress, provide counseling and crisis intervention, and provide individual case management and group sessions. This role will provide both in-person and telehealth care. You will develop comprehensive care plans for clients, measure their progress, identify client needs & problems, and engage in crisis management where needed.

 


We’re also looking for someone who’s good at taking notes, able to prepare clinical documents, and loves working with groups and individual clients. Individuals with a background in addiction treatment or human services strongly preferred.

 


This position works on-site Monday - Friday, with a typical work schedule of 11 AM to 8PM. The OP Case Manager maintains a presence with the current clients and provides regular in person groups to build rapport with their future clients when they transition to OP level of care. This role requires a high degree of independence and ownership, since you are often solving problems virtually and connecting with clients that are remote.

 


What you'll do

 


Clinical Competence

  • Proficient in all counselor responsibilities, including screening,assessment, individualized treatment planning, crisis intervention, group and individual counseling, case management, referral, discharge summaries, clinical documentation, and professional ethics.
  • Utilize agency assessment tools in KIPU for comprehensive client evaluations
  • Identify and prioritize patient needs and issues

Person-Centered Care Planning

  • Assess client needs, develop personalized treatment plans, and coordinate services to address mental health, medical, and social needs for your caseload. Develop individualized care plans focusing on client-reported goals
  • Conduct sessions using evidence-based practices
  • Document accurate client notes and progress in SOAP notes submit in a timely manner
  • Prepare effective discharge plans and transition notes

Collaboration and Communication

  • Collaborate with other case managers, clinicians, housing and support staff
  • Coordinate with health insurance for utilization reviews
  • Participate in clinical treatment team meetings
  • Provide accurate assessment and referral services
  • Communicate effectively with probation officers, lawyers, and family members

Professionalism and Compliance

  • Effectively and effortly utilize and navigate all the software need
  • Ensure accurate and timely clinical documentation
  • Prioritize safety of staff and clients

Uphold and Enforce Standards, Policies, and Ethics

  • Ensure compliance with The Forge Recovery Center’s standards, policies, and code of ethics
  • Uphold client rights and professional conduct
  • Adhere to ethical and legal standards
  • Intervene in crisis situations as necessary

Who you are...

Education, Experience, & Skills:

  • Drug and Alcohol Counselor California Credential: RADT, RAC, SUDRC required. or CADC I, SUDCC, CATC I or above preferred
  • 2-5+ years of Case Manager Experience in SUD or Mental Health.
  • Previous experience with telehealth is preferred but not required.
  • Strong computer skills to utilize software for telehealth platforms and documentation
  • Passion for client focused work and continuous improvement growth of a telehealth community

Additional Certificates & Requirements:

  • CPR/First AID
  • Valid Driver’s License
  • Negative TB test results within past year

 


Job Type: 
Full-time

Pay: $21.00- $27.00 per hour

The pay range for this role is:
