I. Job Summary

The Patient Navigator (Medical) will champion the needs of patients by tracking and following up on all specialty care referrals and authorization requests.

*This position is grant-funded and contracted through November 30, 2025.

II. Essential Functions

1. Obtain prior authorizations, where applicable, for specialty care services/labs/imaging exams in a timely manner.

2. Ensure specialty care referrals are tracked and followed up on.

3. Communicate with insurance carriers to clarify authorization criteria and track prior authorization status.

4. Follow up with patients to ensure specialty care referral appointments are scheduled and kept.

5. Educate patients on access to and utilization of specialty care services.

6. Obtain labs/imaging results and progress notes and upload to eCW.

7. Acts as a liaison between patients, clinic providers, specialty care providers, and healthcare plans.

8. Document services provided for each patient and track completed referrals and referrals that are still outstanding.

9. Provide patient updates to Program Coordinator and Project Manager.

10. Perform additional duties as assigned.

IV. Education/License/Certification:

1. Bachelor’s degree preferred. Medical Assistant certification preferred.

V. Skills and Specifications:

1. Previous experience in a medical office setting required.

2. Knowledge of medical terminology, ICD-10 and CPT codes are a plus.

3. Familiarity with eClinicalWorks preferred.

4. Ability to work collaboratively with staff and patients from diverse socio-economic, ethnic, and cultural backgrounds.

5. Communicate effectively orally and in writing.

6. Demonstrate maturity, good interpersonal skills, integrity, and professionalism when working with patients and team members.

7. Maintain strict adherence to HIPAA guidelines and confidentiality with patient information.

8. Ability to speak, write, and read English fluently.

9. Fluency in a second language (Spanish, Mandarin, or Cantonese) is preferred.

10. Ability and willingness to work some evening and weekend hours.

11. Proficiency with Google Suite and Microsoft Office.

VI. Physical Requirement

1. Physical effort which may include occasional light lifting to a 25 pound limit, and some bending, stooping or squatting. Considerable walking may be involved. The ability to sit or stand for extended periods of time is required.

3. While performing the duties of this job, the employee may be required to work at all HCHC facilities as needed.

Job Type: Full-time

Pay: $22.00 - $26.00 per hour
