Responsible for assisting and carrying out a variety of clinical and program services. This person plays an essential role in the assurance of the admitting process, orientation to new clients and over-all client management. Assists in the admitting process of both residential and day treatment/care clients.
Evaluates prospective clients to prior, employing intervention techniques, interviewing and assessment skills. Demonstrates ability to make an assessment based upon the information gathered.
Obtains pertinent information for the newly admitted client.
Answers questions and educates the client and family members to the philosophy, visiting, guidelines and confidentiality policies.
Assures that all Informed Consents and releases of information are signed.
Searches personal belongings for contraband and retains any electrical equipment.
Assures that client rights are signed and reviewed with newly-admitted client.
Provides a basic orientation to the physical surroundings including their room and introduces client to peers.
Makes appropriate entry in the client record.
Responsible for daily program tasks which provide optimal maintenance and operation.
Maintains complete documentation in the EMR.
Prepares EMR for proper documentation.
Assists in communication and facilitate phone calls with client as approved by the clinical team.
Screens visitors utilizing client confidentiality requirements as criteria.
Assists in appropriate documentation procedures and oversees portions of other documentation with supervision.
Reviews active client files on a weekly basis.
Communicates client’s progress through written documentation in the client progress notes and verbal interaction with the Case Manager.
In cooperation with the recovery team, performs discharge planning, coordinates referrals and follow-up with clients to ensure adequate post-discharge services for continued individual progress.
Conducts group sessions under the supervision of the Case Managers per program standards.
Provides didactic group sessions as directed i.e. orientation group, discharge planning group, etc.
Conducts all group sessions and/or client-staff interaction in a professional and therapeutic manner, placing the clients dignity and rights above personality differences.
Keeps Case Managers informed regarding tentative and/or current problems with clients.
Attends all meetings as required. Maintains professional relationships.
Current CPR and First Aid training.
Maintain registration/certification.
