Work Location: MLK Community Hospital

  • Certification with one of State of California's Alcohol & Drug agencies (CATC, CADC, LAADC, or equivalent California certificate or license) preferred.
  • Bachelor’s degree in addiction studies or behavioral health related field required, associate’s degree can be considered in lieu of experience

POSITION SUMMARY

The Substance Use Navigator (SUN) helps patients access evidence-based treatment for substance use disorders with a focus on low-threshold access to MAT (medication for addiction treatment) and harm reduction. Reporting to the Manager of Social Work, the SUN is responsible for providing: comprehensive substance use assessments, individualized addiction treatment plans, and supportive counseling to MLKCH patients identified by the interdisciplinary team as needing specialized substance use intervention. The role reflects appropriate knowledge of addiction treatment, mental health diagnoses, discharge planning and, case management. The SUN is also responsible for meeting any and all other state requirements, CMS Conditions of Participation, EMTALA, The Patient Bill of Rights, AB1203, and other Federal or State regulatory agency requirements. This position will be supporting the inpatient and emergency room team to ensure high-quality care, and assist in transitioning patients to an appropriate level of care once discharged (Intensive Outpatient Programs, Outpatient Programs, and Inpatient Rehabilitation, sober living, and/or detox facilities). This position requires full understanding and active participation in fulfilling the Mission of Martin Luther King, Jr. Community Hospital. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Martin Luther King, Jr. Community Hospital’s strategic plan and the goals and direction of the quality and performance improvement process activities. The Watson Model of Care is utilized throughout the delivery system of care services.

POSITION REQUIREMENTS

A. Education

  • Bachelor’s degree in addiction studies or behavioral health related field.

B. Qualifications/Experience

  • Ability to work in a fast-past environment, providing brief intervention and linking patients to resources.
  • Knowledge of case management and discharge planning preferred, not required.
  • Nonjudgemental, energetic, positive, harm-reduction approach to assisting patients with Substance Use Disorder (SUD).
  • Substance Abuse Experience Required (minimum 2 years) in an inpatient or outpatient setting providing direct care to patients/families. Hospital or Mental Health clinic experience preferred.
  • Certification with one of State of California's Alcohol & Drug agencies (CATC, CADC, LAADC, or equivalent California certificate or license) preferred.
  • Knowledge and understanding of evidence based interventions and motivational interviewing.
  • Experience using assessment tools such as the BAM, AUDIT, DAST, and CAGE.
  • Ability to educate patients about signs, symptoms, and treatment regarding substance abuse.
  • Knowledge of the relationship between substance use and physical/chronic health conditions.
  • Able to navigate the acute care setting and successfully connect with outside provider networks that will facilitate transitions of care for patients post discharge.
  • Understanding substance use disorder as a medical condition and the role of MAT in treatment and harm reduction. Adherence to an abstinence-based social model that is unsupportive of MAT is not aligned with the goals of this position.

C. Special Skills/Knowledge

  • Bilingual language skills preferred (Spanish)
  • Basic computer skills
  • Current Basic Life Support (BLS)
  • Must complete annual Workplace Violence Prevention Program/Certificate, per hospital policy, during initial training/orientation but not to exceed 30 days from hire/transfer.
