POSITION SUMMARY
The Substance Use Navigator (SUN) helps patients access evidence-based treatment for substance use disorders with a focus on low-threshold access to MAT (medication for addiction treatment) and harm reduction. Reporting to the Manager of Social Work, the SUN is responsible for providing: comprehensive substance use assessments, individualized addiction treatment plans, and supportive counseling to MLKCH patients identified by the interdisciplinary team as needing specialized substance use intervention. The role reflects appropriate knowledge of addiction treatment, mental health diagnoses, discharge planning and, case management. The SUN is also responsible for meeting any and all other state requirements, CMS Conditions of Participation, EMTALA, The Patient Bill of Rights, AB1203, and other Federal or State regulatory agency requirements. This position will be supporting the inpatient and emergency room team to ensure high-quality care, and assist in transitioning patients to an appropriate level of care once discharged (Intensive Outpatient Programs, Outpatient Programs, and Inpatient Rehabilitation, sober living, and/or detox facilities). This position requires full understanding and active participation in fulfilling the Mission of Martin Luther King, Jr. Community Hospital. It is expected that the employee will demonstrate behavior consistent with the Core Values.
