Full Job Description

JOB PURPOSE: The Case Manager is responsible for providing direct services and supportive contacts to the client and/or their families, and developing resources within the community, which are supportive to the recovery of the client. He/she also functions as an advocate for the client, seeing that appropriate services are provided and assists the client in receiving these services as appropriate.

A.

Job Duties and Responsibilities

1. Provide direct clinical services (i.e., assessment, consultation, individual/group/family counseling, substance abuse counseling, and case management, etc.).

2. Maintain clinical documentation as established by SADDLEBACK RECOVERY policy and procedures.

3. Work cooperatively with other community resources and referral sources to coordinate services for clients.

4. Assist in the assessment of needs for mental health, medical, and other supportive services.

5. Good writing skills which enable one to accurately chart client progress, medical necessity and interaction during contact.

6. Communicate clearly and assertively with clients and staff in order to maintain program structure.

7. Lecture in a large and/or small group setting.

8. Participates in SADDLEBACK RECOVERY 24-hour Emergency Services, which includes availability for back-up rotation coverage for after hours, weekends, or holiday emergencies.

9. Carry out other responsibilities as delegated.

2. ATTENDANCE AND PUNCTUALITY - Is prompt and on time for work, assignments and meetings. Notifies supervisor prior to lateness or absence. Understands that excess absences pose a hardship on other employees.

3. JOB SKILLS AND KNOWLEDGE - Employee completes all chart related documents and correspondence for assigned clients in a timely manner. Employee has a thorough understanding of the 12 Core Functions, ASAM criteria, and Stages of Change.

4. Employee provides individual counseling, case management, skills training, and facilitates educational, skill development and counseling groups as assigned and properly and promptly charts these activities in client records

5. Employee works cooperatively with other community resources and referral sources to coordinate services to clients.

6. QUANTITY AND QUALITY OF WORK – Cares about the quality and accuracy of work being produced. Accepts responsibility for completing job tasks. Uses time productively and efficiently.

7. ATTITUDE – Demonstrates a cooperative, positive, and enthusiastic attitude toward fellow employees, clients and visitors. Maintains a positive attitude in carrying out assignments and is helpful and courteous to fellow employees.

8. CONFIDENTIALITY - Know and follows confidentiality law and procedures at all times.

9. COMMUNICATION SKILLS-Is effective in written and oral expression. Demonstrates proper telephone etiquette and able to communicate easily with clients, referral sources, and visitors in an appropriate manner. Relates to clients and co-workers in a respectful and professional manner.

10. CONDUCT – Conducts one’s self in a manner consistent with the agency’s code of ethics and code of conduct. Shows professionalism in office setting.

11. FLEXIBILITY / HANDLING EMERGENCIES – Responds well to changing situations or routines. Handles crisis situation calmly and professionally.

Competencies

To perform the job successfully, an individual should demonstrate the following competencies to perform the essential functions of this position.

· Analytical—the individual synthesizes complex or diverse information in therapy sessions with clients and/or family programming. Case manager recognizes the social, political, economical, and cultural context within which addictions and substance abuse exists

· Problem solving—the individual identifies and resolves problems in a timely manner and gathers and analyzes information skillfully.

· Oral communication—the individual speaks clearly and persuasively in positive or negative situations, demonstrates group presentation skills and conducts productive meetings. Case manager communicates established diagnostic criteria for substance use disorders and treatment modalities within the continuum of care.

· Delegation—the individual delegates work assignments, gives authority to work independently, sets expectations and monitors delegated activities.

· Leadership—the individual inspires and motivates others to perform well and accepts feedback from others during weekly treatment team meetings.

· Management skills—the individual includes staff in planning, decision-making, facilitating and process improvement; makes self-available to staff; provides regular performance feedback; and develops subordinates' skills and encourages growth.

· Quality management—the individual looks for ways to improve and promote quality and demonstrates accuracy and thoroughness. Case manager adapts practice to the range of treatment settings and modalities.

· Judgment - the individual displays willingness to make decisions, exhibits sound and accurate judgment, and makes timely decisions.

· Planning/organizing—the individual prioritizes and plans work activities, uses time efficiently and develops realistic action plans.

· Safety and security—the individual observes safety and security procedures and uses equipment and materials properly.

Minimum Qualifications

Education / Experience

· CADC-1, SUDRC or RADT-1 gathering hours towards certification/preparing for certification exam

· Has a minimum of 2 years of experience in the treatment of behavioral health/ substance abuse disorder.

· Drug and Alcohol Counselor, or enrolled in school for same

· Understanding of counseling and other treatment procedures for alcoholism and drug addiction related problems.

· A vast knowledge of family dynamics that occur within families who have a member who struggles with alcoholism and drug addiction.

· Working knowledge of alcoholism and drug addiction and treatment procedures for such illness.

· Will work in partnership with other staff in achieving and maintaining a therapeutic environment for clients.

· A Case Manager will have the ability to deal with a variety of situations and roles, to handle stress and crisis, and interact with clients of varied temperaments with a professional demeanor.

· Case Managers will need to be self-directed and highly motivated.
