What are the qualifications? (Must Have)
High school diploma or GED + 2 years of mental health experience OR Associates Degree + 1 year of mental health experience OR Bachelors Degree in Social Sciences
One (1) year of providing substance use services
You must have your own vehicle, willing to use it for work and willing to transport members in your car. You will be reimbursed for all your miles driven for the job.
You must have a driver's license that is valid and current. Must have an acceptable Motor Vehicle Report Background Check and meet the driving standards of Telecare's Fleet Safety Manual
You must have valid and current car insurance with your name listed as a driver.
Experience in San Diego County EHR Cerner is a big plus!
You must want to work Monday - Friday, 8:00am - 5:00pm.
Must be willing to rotate a Crisis After Hours cell phone with the rest of the staff (typically employees take the crisis cell phone home, at minimum, 4 times per year)
What will the job entail?
The Case Manager Substance Use Specialist provides members with community support and case management services using Psychiatric Rehabilitation principles, Recovery Philosophy, and Evidence-Based practices. This position provides housing support to members served and their families. You will coordinate service needs and collaborates with other services and agencies such as Substance Use Groups, AA, NA, Conservator's Office, Public Defender's Office, Child and adult Protective Services, Probation Department, and Inpatient Services, Long Term Care Services, Residential Settings and County Mental Health Services. The Substance Use Specialist provides case management services to ensure all clients receive co-occuring alcohol and other drug therapy, use Stages of Change to set and complete client goals, encourage harm reduction methods, visit AA or NA, completes assessments, provides mental health rehabilitation services, case management, crisis intervention, integrated whole person care, and trauma informed care. Each day you will be required to provide 5.5 hours of member/client care. Most of those services will occur in the community. The other 2.5 hours each day are reserved for Team Morning Meeting, administrative needs, training, coaching with your supervisor, completing other assigned tasks.
